On behalf of Summerville Family Health Team, we are pleased to present our Annual Report for 2023-2024. Since being established in 2007 as one of the first Family Health Teams in Ontario, Summerville has been striving to provide people-centred, high-quality, integrated, and accessible primary care in our community. 2023-2024 has been a great year and we have many highlights to showcase!
